We refuse to increase our fees due to such programs that eventually get passed on to the patients that do not participate in managed care programs.

Most insurance companies base their payments on either a set fee schedule or "Usual & Customary" (U&C) amounts. Usual & customary charges are determined at the time the insurance policy is written for the employees within an employer group. These pre-determined charges are based on an average charge for that specific service from the dental offices within that specific area. When an insurance policy is being written, the employer can choose to purchase coverage for a percentage of the pre-determined "usual & customary" charges. U&C charges are typically not released from the insurance company until a claim is considered.
